Test requests

Students are responsible for entering information on the test request form accurately. Test Centre staff are not responsible for information that is incorrect resulting in tests not arriving at the Test Centre on time, rescheduling of tests and delaying the test request process.

Please fill out one form for EACH exam you are booking. Please do not put multiple dates on one form.
