Missed test request form

To schedule a missed on-campus test, please complete the form below. You must receive approval from your faculty to make up your test prior to completing this form. All requests must be submitted at least seven (7) days in advance of the proposed test date. Testing staff will reply to your request in the order it was received. Do not send multiple requests for the same test.
