Pre-Approved Missed or Alternative Date Testing Form

These tests are permitted to be written in the Testing Centre only in circumstances in accordance with Academic Regulations 10.1.2. Inclement weather, vacations, work schedules, or other reasons not listed in the Academic Regulations will not be approved in the testing services.

You must receive approval from your faculty prior to completing this form. All requests must be submitted at least seven (7) days in advance and is subject to availability at the campus. For each test you require to be scheduled, a separate form is required. No missed or alternative date tests will be booked during mid-term exams and final exam weeks.
